The regions of the world are based on "List of Countries by
Region" from the CDC publication "Health Information for
International Travel 2003-2004." Each country of the world is assigned to
one of 16 regions. The regions and countries are also listed on the CDC
Traveler's Health website: http://wwwn.cdc.gov/travel/destinationList.aspx

If the patient lives in the United States and has not traveled, limit your query
to diseases endemic to North America. If the patient has traveled recently, add
all visited countries as criteria in an OR search (Any of these).
